Résumé / Abstract : Le diverticule de Kommerell décrit pour la première fois en 1936, correspond à une anomalie vasculaire congénitale très particulière et s’intègre dans les anomalies encerclante des arcs aortiques. Il peut être à l’origine d’une artère sous-clavière dite « aberrante ». Cette pathologie rare forme avec le ligament artériel, un anneau vasculaire encerclant la trachée et l’œsophage. Les patients présentent le plus souvent des symptômes respiratoires et digestifs tels que la dyspnée, les infections respiratoires répétées, le stridor ou la dysphagie. L’absence d’intervention chirurgicale expose aussi le patient à un risque de dissection et de rupture du diverticule pouvant engendrer le décès. La résection de celui-ci, la division du ligament artériel et la réimplantation de l’éventuelle artère sous-clavière « aberrante » sur la carotide commune sont actuellement préconisées. Objectif. Décrire la symptomatologie, les examens complémentaires réalisés, la méthode chirurgicale, les complications et l’évolution post-opératoire chez une population de 13 patients et montrer l’importance de la chirurgie devant le risque de dissection et de rupture du diverticule de Kommerell. Matériel et résultats. Entre 8/2007 et 10/2016, 13 patients consécutifs ont subi une intervention chirurgicale. La symptomatologie, les examens complémentaires ayant permis le diagnostic, la procédure chirurgicale, les complications, l’analyse histopathologique et les résultats cliniques ont été analysés rétrospectivement. L’âge moyen au moment de la découverte était de 9.6 ans (médiane 6.6 ans, intervalle 0.1–36.8) avec une chirurgie en moyenne à l’âge de 10.5 ans (médiane 6.7 ans (0.5–37.1). Le poids moyen était de 28.7 Kg (médiane : 20.5, [6.6-85]). Le suivi moyen des patients était de 25 mois (médiane : 8 mois, (0.5–96 mois). Tous les patients étaient symptomatiques, 10 d’entre eux présentaient des symptômes respiratoires, 4 patients avaient des symptômes digestifs et un patient a présenté un épisode de vol sous-clavier avec œdème du membre supérieur gauche. L’angiographie par tomodensitométrie et/ou l’imagerie par résonance magnétique ont permis de faire le diagnostic précis. Les complications post-opératoires répertoriées sont un chylothorax chez un patient, une pneumopathie chez 3 d’entre eux dont 2 associées à un pneumothorax. L’analyse histopathologique réalisée chez 6 patients retrouve 3 prélèvements avec signe de média-nécrose kystique, soit 50%. Le suivi post-opératoire a permis de montrer une amélioration des symptômes chez 8 patients et une disparition complète de la symptomatologie chez les 5 autres. Conclusion. La résection du diverticule de Kommerell avec réimplantation de l’artère sous-clavière et division du ligament artériel est une chirurgie préconisée. Pour diminuer la symptomatologie, la simple section du ligament artériel est suffisante. Au vu du risque de dissection et de rupture du diverticule et à certaines persistances et/ou récurrences de symptomatologie, nous encourageons cette première stratégie chirurgicale, d’autant plus que le risque de complications post-opératoires observé est minime.

Résumé / Abstract : Background. Kommerell’s diverticulum, first described in 1936, is a very particular congenital vascular abnormality and can lead to the formation of a vascular ring or an aberrant subclavian artery. In this rare condition, a vascular ring forms with the ligamentum arteriosus, which encircles the trachea and the esophagus. Patients most often have respiratory and digestive symptoms such as dyspnea, recurrent respiratory infections, stridor or dysphagia. Leaving a Kommerell diverticulum in place exposes the patient to a risk of dissection and rupture of the diverticulum that can be fatal. The treatment currently advocated involves resection, division of the ligamentum and translocation of any aberrant subclavian artery to the common carotid artery. Objectives. To describe the symptoms, workup, surgery, postoperative events and follow-up of a group of 13 patients and to demonstrate the importance of surgery in lowering the risk of dissection and rupture of Kommerell diverticula. Methods. Thirteen consecutive patients who received surgical treatment for a Kommerell diverticulum between August 2007 and October 2016 were included in the study. Their symptoms, workup, the surgical procedure used and any complications, histopathological findings, and the clinical outcomes were analyzed retrospectively. Results. The mean age at diagnosis was 9.6 years (median, range: 6.6 years, 0.1-36.8 years) and the average age at surgery was 10.5 years (6.7 years, 0.5–37.1 years). The patients weighed 28.7 kg on average (20.5 kg, 6.6–85 kg). The mean follow-up period after surgery was 25 months (8 months, 0.5–96 months). The patients ail presented at least one of the following symptoms; 10 had respiratory symptoms, 4 had digestive symptoms, and one patient had suffered edema of the upper left limb. Computed tomography and/or magnetic resonance imaging confirmed the diagnosis for ail the patients. Postoperative complications included chylothorax (1 patient), pneumonia (3 patients) and pneumothorax (2 patients). Histopathological analysis of the diverticulum after resection revealed cystic medial necrosis in two patients (33%). Symptoms improved for eight patients after surgery and disappeared for the five others. Conclusion. The recommended treatment for Kommerell’s diverticulum is resection with translocation of the subclavian artery and division of the ligamentum arteriosus. A simple section of the ligamentum is sufficient to reduce the symptoms. Given the risk of dissection and rupture of the diverticulum and some persistence and/or recurrence of symptoms, we recommend this more simple operation, especially since the associated risk of postoperative complications recorded in this study is minimal.
