Résumé / Abstract : OBJECTIF. La fréquence et de la sévérité des troubles urinaires dans le syndrome de Gougerot-Sögren est suggérée dans plusieurs séries. Ces troubles restent méconnus et peu recherchés en pratique courante. L'objectif de cette étude est de décrire une typologie clinique et urodynamique des troubles vésico-sphinctériens dans le syndrome de Gougerot-Sjögren. METHODE. - Analyse rétrospective analytique et observationnelle, multicentrique dans trois centres hospitaliers, étudiant les patients venus consulter entre 1993 et 2015 pour des troubles vésico-sphinctériens et ayant eu un bilan urodynamique. Les critères d'exclusion étaient personne mineure, lésion neurologique sans lien avec le syndrome de Sjögren, antécédent de radiothérapie ou chirurgie ou traumatisme pelvien, anomalie morphologique urinaire, traitement diurétique ou anticholinergique. RESULTAT. — Cinquante-quatre patients ont été inclus, de sexe exclusivement féminin. L'hyperactivité vésicale prédominait chez 74,1%, associée à une incontinence urinaire chez deux tiers des patients (p=0,001). Parmi les 68,5% de patients incontinents, la répartition était 43% mixte, 30% d'effort et 27 % sur urgenturie. Des troubles obstructifs étaient décrits chez 46,3% des patients, avec un risque de résidus post-mictionnels (p=0,04) et d'infections L'association de symptômes irritatifs et obstructifs était retrouvée chez 37 % des patients (p=0,001). La plupart des patientes (61,1%) avait des anomalies urodynamiques. CONCLUSION. - Les troubles vésico-sphinctériens peuvent être reconnus comme des manifestations associées au syndrome de Gougerot-Sjögren et leur dépistage doit être systématique. L'association des symptômes et l'intrication des mécanismes physiopathologiques justifient une consultation spécialisée précoce pour optimiser la prise en charge diagnostique et thérapeutique.
Résumé / Abstract : OBJECTIVE. The frequency and severity of urinary symptoms in Sjogren's syndrome is suggested in several series. These disorders remain unknown and few sought in routine. The objective of this study was to describe clinical and urodynamic typology of urinary disorders in Sjogren's syndrome. MATERIAL AND METHOD. - Retrospective, observational, multicenter analysis, studying patient came between 1993 and 2015 refer to the urinary disorders and who had an urodynamic investigation. Exclusion criteria were: minor, unrelated neurological injury Sjogren's syndrome, history of pelvic radiotherapy or surgery or trauma, urinary morphological abnormality, anticholinergic or diuretic therapy. RESULTS. Fifty-four patients were included, of exclusively female. Overactive bladder predominated at 74.1%, associated with urinary incontinence in two thirds of patients (p=0.001). From 68.5% of incontinent patients, the distribution was 43% mixed, 30% strain and % of urge incontinence. Obstructive disorders were reported in 46.3 % of patients, with the risk of post- void residual urine (p=0.04) and infection (p=0.04). The combination of irritative obstructive symptoms was found in 37% of patients (p=0.001). Most patients (61.1%) had urodynamic abnormalities. CONCLUSION. - The lower urinary tract symptoms can be recognized as events associated with Sjogren's syndrome and screening must be systematic. The combination of symptoms and the intricate pathophysiological mechanisms justify an early specialist consultation to optimize the diagnostic and therapeutic management.
