Résumé / Abstract : Introduction : En 2009, les Agences Régionales de Santé ont imposé aux réseaux de santé un nouvel outil de coordination, le Plan Personnalisé de Santé (PPS). Cette étude explore les perceptions et les pratiques des acteurs de santé mentale autour du PPS du RéHPI (Réseau Handicap Psychique de l’Isère). L’objectif est de proposer des améliorations pour le PPS et/ou son utilisation. Méthodologie : Les données ont été obtenues par onze entretiens individuels menés auprès de membres actifs du RéHPI. Elles ont été exploitées selon les techniques d’analyse thématique. Résultats : On constate une variabilité dans les pratiques et surtout les perceptions des acteurs de santé mentale autour du PPS. Discussion : Cette variabilité est plus à mettre en lien avec la proximité affective des acteurs envers le RéHPI qu’avec l’appartenance à des référentiels différents. Les modifications à venir concernent une rubrique Aidants et l’intégration des soins somatiques aux soins curatifs. La qualité de la transmission orale accompagnant le PPS conditionne la perception du sens de cet écrit pour les acteurs.

Résumé / Abstract : Introduction: In 2009, the Regional Agencies of Health imposed on the networks of health a new tool of coordination, the Health Personalized Plan (PPS). This study investigates the perceptions and the practices of the actors of mental health around the PPS of the RéHPI (Psychic Handicap Network of Isère). The objective is to propose improvements for the PPS and\or its use. Methodology: the data were obtained by eleven individual interviews led with active members of the RéHPI. They were exploited according to the techniques of thematic analysis. Results: we notice a variability in the practices and especially the perceptions of the actors of mental health around the PPS. Discussion: this variability is more to put in connection with the emotional nearness of the actors to the RéHPI than with the membership in different reference tables. The modifications to come concern a section Aidants and the integration of the somatic care in the curative care. The quality of the oral transmission accompanying the PPS conditions the perception of the sense of this paper for the actors.
