Résumé / Abstract : La spondylarthrite (SpA) est l'un des deux rhumatismes inflammatoires chroniques les plus fréquents en France. C'est une maladie complexe, déterminée par plusieurs facteurs génétiques et environnementaux. Si l'allèle HLA-B27 est présent chez 80% des patients, d'autres facteurs génétiques prédisposent aussi à la SpA. Il existe notamment une région de susceptibilité sur le chromosome 9q31-34 (SPA2). Le but de mon travail a été de caractériser le rôle joué par cette région dans la prédisposition à la SpA. L'étude systématique des gènes candidats de SPA2 nous a permis d'exclure l'implication directe de 10 candidats majeurs. Par cette stratégie nous avons également identifié un haplotype de 3 SNPs significativement associé à la SPA dans le gène C9ORF91. En parallèle, une étude conjointe de liaison et d'association de type région candidate nous a mené à identification d'un haplotype de 6 SNPs très associé à la SpA et localisé au voisinage d'un des meilleurs gènes candidats de SPA2, TNFSF15.
Résumé / Abstract : Spondyloarthritis (SpA) in one of the most frequent varieties of articular inflammatory disorders in France. It is a complex disorder, depending on several genetic and environmental factors. Although HLA-B27 allele is present in 80% of patients, other genetic factors are also implicated in SpA predisposition. Notably, there is a susceptibility region located on chromosome 9q31-34 (SPA2). The goal of my thesis work was to characterize the role played by this locus in the SpA predisposition. Systematic candidate-gene association studies allowed us to exclude direct implication of 10 major candidates. This strategy also led us to the identification of a 3-SNPs haplotype significantly associated with SpA within the C9ORF91 gene. In parallel, a comprehensive linkage and association region-candidate study allowed us to identify a 6-SNPs haplotype strongly associated with SpA and located in the vicinity of one of the major SPA2 candidate gene, TNFSF15.
