Résumé / Abstract : Drug-induced iatrogenesis is common in elderly patients taking multiple medications, and the risk increases with the introduction of oral anticancer drugs (OADs). The aim of our study was to evaluate the impact of clinical pharmacists in preventing this iatrogenesis in oncogeriatrics at the Toulouse University Hospital. This retrospective observational study included patients who underwent a drug interaction (DI) analysis at the start (M0) of OAT. Treatments 3 (M3) and 6 (M6) months after initiation were collected retrospectively. Among 58 patients, 80% had a DI at M0 compared to 30% at M3 and M6, after intervention by the clinical pharmacist. However, 83% of patients had at least one potentially inappropriate prescription, a prevalence that remained stable over time. We thus demonstrated that the clinical pharmacist contributed to ensuring the safe initiation of oral therapies in oncogeriatrics. However, limitations remain in the reassessment of chronic treatments during follow-up, highlighting the need for better structured multidisciplinary work and a stronger link between community and hospital settings, particularly with community pharmacists, whose role in these complex care pathways and in overall prevention has been proven but is often still underutilized.
